We are pleased to share with you that we have officially filed notice and submitted a Definitive Agreement between SolutionHealth and Home Health & Hospice Care (HHHC) with state officials, to combine services to provide enhanced post-acute care services to the people of southern New Hampshire.
The proposed affiliation centers on HHHC (including the Community Hospice House) in principle operating as part of SolutionHealth, alongside Elliot Health System, the VNA of Manchester and Southern New Hampshire, and Southern New Hampshire Health and developing a fully integrated, regionally-coordinated system of post-acute care.
We announced a Letter of Intent about this endeavor in early February of this year and have spent the last several months conducting due diligence, and defining the parameters for this new, combined service. This agreement now moves to the regulatory approval phase with the Charitable Trusts Unit of the New Hampshire Attorney General’s office.
The affiliation will allow for increased access to clinical expertise, seamless information technology, and new and innovative programs and services, all utilizing the expertise of two of the region’s most experienced home health agencies as a foundation. Our goal is to provide a comprehensive suite of services to ensure access to high-quality post-acute care throughout southern New Hampshire.
For our patients and providers, this partnership will offer many benefits, including an integrated medical record which will close care gaps and improve patient outcomes, expanded access to care, greater clinical specialty sophistication, increased population health offerings, and improvements to clinical and operational efficiency. The trend toward advanced care outside of the acute setting has been advancing for years, and the pandemic has only accelerated that trend and an increase in innovations in, and demand for, care at home.
The regulatory process could take several months to complete and will include at least one public hearing where we will provide information and gather further feedback from our community. Information about this forum will be shared once available.
