The Visiting Nurse Association of Manchester and Southern New Hampshire works collaboratively with Elliot Health System and its parent company, SolutionHealth, to provide exceptional hospice services to those at home, including those residing in skilled nursing facilities and assisted living.
In this video, Jennifer Rydeen, MSW, a hospice social worker at the VNA of Manchester and Southern New Hampshire, explains the special bonds formed with her patients.
“A lot of times I tell my patients we come in as strangers, we then become a team, and we sometimes become like family because we are providing that compassionate care for them that’s intimate in their own home,” she says.
The VNA of Manchester and Southern New Hampshire is made up of nurses, social workers, licensed nursing assistants, chaplains, physicians, and volunteers. They all work together to ensure people with a life-threatening diagnosis have a good quality of life for the time remaining.
