Elliot Health System’s Partial Hospitalization Program (PHP) is helping our community members who are battling both mental health issues and substance use disorder.
The PHP includes a multidisciplinary team made up of licensed mental health and substance use clinicians, psychiatric providers, registered nurses, case managers, and recovery support staff who closely coordinate care with primary care providers.
All program participants are assessed with a variety of standardized tools. Treatment plans are then developed for that individual to not only address their substance use and mental health issues/needs, but also the major social determinants of health. Patients receive intensive individual and group treatment, followed up by strong aftercare and family components to help ensure the continuity of care and the inclusion of family and support members in the treatment process.
